Application for Credential Evaluation
Complete the Credential Evaluation Application if you:

Previously submitted but did not complete an application and were notified that your application was destroyed after 18 months. (You must again pay the full credential evaluation fee.) |
The initial evaluation fee of $400 covers the review, authentication, and preparation of two evaluation reports. The licensing board receives one report electronically, and the other report is mailed to the applicant as a hard copy.
Application Process
A complete application consists of:

Upon receipt of a completed application, the Center will:

Acknowledge receipt of your application (electronically). |
| • |
Assign an application number. (Keep this number for future correspondence and actions. If you request academic documentation after your application has been filed, please state your file number on the Transcript Request Form and all correspondence with the Center.) |

Review your application for accuracy. |
| • |
Notify you of all records needed to complete your review and evaluation. |
| • |
Notify you of the status of your application at 45-day intervals, for three consecutive periods. You can track the progress of your application at any time by visiting https://commerce.ncees.org/credentials/eval_status. |

Complete the evaluation of your file when all required documentation is received and degrees verified. |
Upon completion of an evaluation, the Center will:

Transmit one electronic report to the licensing board of your choice. |
| • |
Send one hard copy to you by regular U.S. Postal Service at the address listed on the application, unless otherwise authorized by you. |

Application Instructions

Forms are provided in an online format and can be processed and submitted for payment via credit card. The secure transaction is encrypted via SSL. |

If you prefer to submit payment with a cashier's check or official bank check, please fill out the forms online and submit the data. Print a copy of the confirmation screen and mail it to:

Shortly after your application has been received, you will receive an e-mail from the Center identifying the additional documents you will need to submit. |
| • |
State your name as it appears in official documents. Include any other names or name changes (maiden name, transliterations, etc.). |
| • |
Make sure your mailing address, e-mail, phone number, and fax number are accurate so that the Center will be able to contact you. |
| • |
List in chronological order each postsecondary institute attended and its location (city and country). If you attended an affiliated or autonomous college of a university, please indicate this. |
| • |
List actual years of attendance at all institutions; each intermediate and final degree, diploma, or other qualification earned (use the indigenous term); and the date the degree was awarded. Include advanced placement examinations (A-levels, Abitur, etc.). |
| • |
List all postgraduate degree studies (postgraduate diplomas, master’s, and doctoral degrees). |
| • |
Do not list attendance at short courses, training, or continuing education for which academic degree credit was not granted. |
| • |
A resume or curriculum vitae can not be accepted as a substitute for the application. |
Incomplete Applications
The Center will deactivate your file automatically if it is not completed within 180 days. After 180 days, no further action will be taken on an incomplete application until a reactivation form and required fees are submitted. It will securely destroy the content of your file 1 year after the file has been given deactivation status. New application, applicable fees, and documentation are required for an evaluation that has been destroyed.
Required Documentation
The Center will not complete a credential evaluation until all required and official documentation has been received. Documentation includes the following: • Official Transcripts and Degree Verification

Official Transcripts and Degree Verification

Send the Transcript Request Form to all academic institutions where you completed postsecondary credit courses and from which you received a degree or professional qualification. |
| • |
The official transcripts and academic records must be prepared in the traditional institutional format and official language. (In most countries, official records are issued in the official language of the country.) |
| • |
The official transcripts and records should list all courses attempted, list all grades received, include class hours or credits (lecture and lab), bear the official institutional seals, and be signed by the designated institutional officials. |
| • |
Official verification of degree earned, including date of graduation, name of degree in the original language, and major or concentration, must submitted by the institutions that issued the degree, title, or professional qualification. |
| • |
Institutions should mail all transcripts, degree verifications, and Transcript Request Form directly to the Center in a sealed and stamped envelope. |
| • |
Records verification cannot be performed by the applicant or individuals not charged by the national academic authorities to perform such functions. |
Translations
| • |
An official literal or word-by-word English translation must accompany all records (transcript/degree) issued in the native language. |
| • |
Translations must mirror the official native language record and include all data presented in the original document in the same format and order. |
| • |
Grades should appear as listed in the original document. |
| • |
Translated documents do not constitute official documents and cannot be used for evaluation purposes. However, they must be prepared and signed by a certified translator in the home country or in the USA. |
| • |
All translations must be sent directly to the Center and include contact information. |
Course Descriptions
Applicants should include course descriptions showing the content of courses completed and contact hours if it is not stated in the record. Information about course content is necessary to establish the nature and level of the course, and determine its applicability to the program.
Other Required Documentation
The Center reserves the right to ask for additional information deemed appropriate to complete the evaluation or to contact the academic institutions or translator if necessary. |
